There are four main stages in the life of a claim, and it all starts here. Click \u201cstart a claim\u201d to get your initial instructions about any forms or paperwork you may need to complete. 

If the claim is through your company or insurance company, an information packet may have been sent to you already via mail or email. <\/p>\n<\/div>\n\n\n\n

Fill out any forms or paperwork needed and send back to Sedgwick. Depending on your type of claim, some forms may need to be filled out by a medical provider as well.
